Decades, LLC in Santa Fe and Albuquerque provides critical services to help assess a frail individual’s situation and create an initial care plan. Initial care management services may include a professional geriatric consultation and the development of a care plan.

Professional Geriatric Consultation

A licensed professional who is specially trained in meeting the challenges of aging will spend an hour or two with your loved one in their home or at the offices of Decades, LLC to conduct a Professional Geriatric Consultation. Your loved one’s needs will be discussed at length, and you’ll learn about the options, local services, and resources that are most appropriate for your loved one’s individual situation. Decades, LLC will verbally outline our suggestions for a successful resolution.

Care Planning

Care Planning is a two-hour evaluation with an experienced New Mexico elder care professional. Family members and caregivers are welcomed at this in-person interview which will cover a broad range of topics critical to frail elderly health aspects, including, but not limited to:

  • Everyday activities
  • Nutritional status
  • Safety
  • Cognition
  • Mental status
  • Finances
  • Insurance

Your loved one has specific needs, health issues, and concerns, and in order to help you appropriately plan for their future, Decades, LLC must accurately assess any issues your loved one or you as a care provider or family member are facing. After the assessment, you will receive a written report with detailed findings, recommendations, and an initial care plan that addresses each issue. You’ll receive information about local resources and elder care services. The program can be completed within 14 days plus time for mail delivery.

The Decades, LLC care manager can also meet with the senior and family members to review and discuss the written care plan.

In spite of your initial care plan, frail elderly health aspects and living situations may change over time. The initial care plan is a foundation that may be altered or built upon as needed. Our team of professional consultants and elder care managers will take a holistic view of your aging loved one’s situation and apply any necessary changes to ensure your loved one’s health and well-being.
