Decades, LLC “Springing Client Program” was created to provide you peace of mind in the event of a health emergency, a decline in physical or mental health, or chronic illness. The program enables you to create a support system for you and your family that “springs” into action when you need it.

At Decades, LLC, we understand that not everyone has someone they trust or who is able to make decisions for them if the need arises in the future. That’s what our “Springing” Care Management Program is for. It’s designed to empower you to live your life with confidence that your wishes will be carried out exactly how you intended when you are no longer able to make decisions for yourself.

Under the “Springing Client Program,” you may designate Decades, LLC:

• as your Power of Attorney (Durable and/or Health)
• as the Personal Representative of your estate
• to act as agent for those who are already your Power of Attorney

Whatever you need, and in whatever capacity you would like us to serve in your aging years, we are here for you.

Here’s How it Works:

• You receive an initial appointment at no charge to discuss your needs and goals.
• There is a one-time setup fee of $150 for individuals ($250 for couples) to cover costs associated with gathering personal, medical, financial and legal information.
• You receive assistance with completion of Decades, LLC’s forms, and procuring legal forms from your attorney.
• You are provided with an Emergency Packet of information, including a laminated card for your wallet to be used in the event of a medical emergency.
• Your information will be updated annually after the first year with $50 renewal membership.

Once you’re enrolled in the “Springing Client Program,” you can count on Decades, LLC to be there with our emergency on-call service—available to you 24 hours a day, 7 days a week—in the event you become incapacitated due to illness or accident.
